Job summary

Gloucestershire Health and Care NHS Foundation Trust in Gloucester is recruiting a Home First Planner and Administrator to support the new countywide Home First service launching on 1 September 2026. You will coordinate patient visits, manage service capacity and maintain schedules to enable safe, person-centred care.

Based at the Home First Hub in Brockworth, you will provide diary management, record keeping, and liaison with staff, patients and partner organisations, while maintaining high

Qualifications

• GCSE Maths and English at grade C or above.
• NVQ Level 3 in administration or equivalent/experience.
• RSA Typing Level 2 or equivalent qualification or experience.

Responsibilities

• Coordinate and schedule patient visits to ensure care plans are delivered safely and efficiently.
• Record and maintain information on clinical systems; update schedules and monitor capacity.
• Provide diary management, meeting coordination and records for the multidisciplinary team.
