Job summary

CCH Group, the UK’s largest and most trusted care provider, is seeking a Care Coordinator to join the Abacare team in CF45 4SN. You will coordinate field-based care, manage rotas, and ensure high-quality home care for local clients.

This full-time, permanent role requires a valid driving licence and access to your own vehicle due to travel across the community. NVQ level 3 is desirable; Monday to Friday with on-call duties and no sponsorship available.

Qualifications

• Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.
• Stepping Up? If you have previous healthcare experience and a solid understanding of CQC, care plans, and rotas, we're here to support your transition into this role.
• NVQ level 3 is desirable

Responsibilities

• The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties.
• Assess and review individual care plans.
• Supervise, instruct, and support care staff in the field.
• This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
