CCH Group is seeking a dedicated Care Coordinator to organise and deliver high-quality home care across the community. The role involves managing care workers, reviewing care plans, and supporting field staff, with travel required.

A valid driver’s license and access to a vehicle are essential due to travel involved. This is a full-time, permanent position in Wales with rota-based on-call duties.

Qualifications

• Experience & Organisation: you are an efficient, organized Care Coordinator who excels at rota planning and building strong care worker relationships.
• Healthcare knowledge including CQC, care plans and rotas is valued for transition.
• NVQ level 3 is desirable.

Job Description

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
