Job summary

CCH Group is seeking a Care Coordinator to manage rotas, supervise field staff and plan high-quality community care for clients across our network. You will oversee care plans, coordinate schedules and ensure excellent service while meeting regulatory and safety requirements.

The role requires a valid driving licence and access to your own car, with travel across branches. Monday to Friday 9am-5pm, on-call duties on a rota basis, offering opportunities for growth.

Qualifications

• Experience & Organisation: you are an efficient, organized Care Coordinator.
• Ability to manage rotas to impeccable standards and build strong relationships with care workers.
• NVQ level 3 is desirable.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
