Leaders in Care is seeking an experienced Adult Nurse Case Manager to deliver high-quality complex care in patients' homes across Bristol and North Somerset. This specialist role leverages critical care skills in tracheostomy care, ventilation and clinical governance within community packages.

• NMC Registered Adult Nurse (RGN) with current registration.
• Experience in Intensive Care (ITU/ICU), Critical Care or community complex care.
• Strong knowledge of tracheostomy care and ventilation.
• Full UK driving licence and willingness to travel across Bristol and North Somerset.

Responsibilities

• Manage a caseload of adult complex care packages across Bristol and North Somerset.
• Complete initial clinical assessments and ongoing patient reviews.
• Oversee care for patients requiring tracheostomy care, invasive and non-invasive ventilation.
• Develop and review individualised care plans and clinical risk assessments.
• Provide clinical supervision and support to care teams; deliver staff training.
• Conduct clinical audits and uphold governance standards.

Adult Nurse Case Manager – Complex Care Bristol & North Somerset | £43,000 per annum | Monday–Friday | Full Time | Permanent

Your Role

As an Adult Nurse Case Manager, you'll take clinical responsibility for overseeing complex care packages, ensuring patients receive safe, personalised and high-quality care at home.

Your responsibilities will include:

• Managing a caseload of adult complex care packages across Bristol and North Somerset
• Completing initial clinical assessments and ongoing patient reviews
• Overseeing care for patients requiring tracheostomy care, invasive and non-invasive ventilation
• Developing and reviewing individualised care plans and clinical risk assessments
• Providing clinical supervision, guidance and support to care teams
• Delivering clinical training and assessing staff competencies
• Conducting clinical audits and maintaining high standards of clinical governance
• Working closely with patients, families and multidisciplinary professionals
• Supporting the recruitment, induction and development of care staff

About You

• NMC Registered Adult Nurse (RGN)
• Experience in Intensive Care (ITU/ICU), Critical Care or community complex care
• Strong clinical knowledge of tracheostomy care and ventilation
• Confident assessing and managing patients with complex clinical needs
• Excellent communication, clinical assessment and organisational skills
• Ability to provide clinical guidance and support to care teams
• Full UK driving licence and willingness to travel across Bristol and North Somerset

Previous Nurse Case Manager experience isn't essential. We'd particularly welcome applications from experienced ITU/ICU Nurses looking to move into community care, as well as established Complex Care Nurse Case Managers seeking a new opportunity.
