Responsibilities

• Undertake face-to-face, telephone and digital consultations with patients.
• Assess, diagnose, investigate, treat and manage patients with acute and chronic conditions.
• Prescribe medication safely and appropriately following current guidance and procedures.
• Refer patients to secondary care and other services when clinically appropriate.
• Provide preventative healthcare, screening and health promotion.
• Maintain accurate and contemporaneous clinical records and documentation.
• Develop and manage long-term conditions and complex healthcare needs.
• Participate in partnership governance, clinical leadership and quality improvement.
• Collaborate with the wider multidisciplinary team and external healthcare professionals.
• Contribute to PCN/primary care network initiatives and service development.
