Responsibilities

• Enable People we Support to access activities and respect personal choices.
• Follow person-centred care plans to meet individual needs.
• Assist with personal care, daily living tasks, and maintain a safe environment.
• Complete daily care documentation accurately.
• Administer and record medication safely.
• Support meal preparation and domestic tasks.
• Provide psychological, emotional and physical support.
• Support community access and accompany appointments.
• Report concerns to maintain safeguarding and safety.
• Foster positive relationships with family and professionals.
• Encourage independent living skills and participate in training.
