Job summary

City Health Care Partnership CIC is seeking an Urgent Community Response Call Coordinator to support referrals, advice requests and service enquiries within the Frailty team in Hull. You’ll work with clinicians, care homes and GP practices to ensure timely access to community services.

The role combines customer service, coordination and administration, with attention to patient confidentiality and accurate data recording. A flexible, team-focused approach is essential.

Qualifications

• Qualification in Business Administration or Customer Service or equivalent experience.
• Experience in an office, administration or customer service role.
• Strong communication and organisational skills.
• Good attention to detail and confidence using Microsoft Office systems.
• Flexible and team-focused with commitment to excellent customer service.

Responsibilities

• Act as the first point of contact for healthcare professionals, care homes, patients and carers.
• Coordinate referrals and appointments, ensuring information is processed accurately and promptly.
• Maintain patient records and update electronic systems.
• Provide administrative support, including meeting preparation, minute taking and diary management.
• Work with colleagues to improve processes and support service delivery across the team.
