Job summary

The role focuses on holistic care from assessment to discharge within the community setting and is offered on a full-time basis.

Qualifications

• Evidence of continuing professional development as a registered nurse.
• Experience communicating complex information to patients, carers and health and social care staff.
• Palliative care training including bladder and bowel care and catheterisation.
• Experience of continuing health care assessments.
• Experience of COPD or Heart Failure.
• Understanding clinical supervision and ability to apply to practice.
• Diabetes course.
