Job summary

The Harbour Frailty Service at Newton Abbot Hospital, part of Torbay and South Devon NHS Foundation Trust, seeks a GP to provide senior clinical decision-making and leadership for frailty care, enabling timely assessment and care closer to home.

Responsibilities include frailty coordination, telephone triage, Harbour @ Home virtual ward, SDEC with access to community beds, and contributing to training, development and service improvement across the Harbour team.

Qualifications

• Full GMC registration
• MRCGP or equivalent
• On GP Performer’s List or TSDFT pathway
• Up to date with appraisal and revalidation requirements
• MD or PhD
• Additional qualification relating to frailty and acute care, e.g. Diploma in Geriatric Medicine, MRCP or MSc
• Membership of the British Geriatrics Society

Responsibilities

• Provide clinical telephone triage to referrers and determine the most appropriate care for frail patients.
• Hold clinical responsibility and provide senior decision-making for Harbour team in GP lead's absence.
• Coordinate across Harbour, acute care, UCR, and community services to facilitate timely interventions.
• Deliver CGA-based assessments and management plans for older people living with frailty.
• Contribute to training, supervision, and professional development of Harbour clinicians.
• Support service development, quality improvement, data evaluation and teaching.
