Job summary

NHS England is expanding Frailty Virtual Wards to provide care closer to home for patients who would otherwise be admitted to hospital. The role focuses on compassionate home-based nursing, listening to carers and patients, and delivering high-quality care in partnership with the MDT.

You will work to respect patients' wishes, act as a role model, and contribute to service improvements. A full UK driving licence and access to a vehicle with business insurance are required.
