Job summary

Two Harbours Healthcare Ltd is seeking a Health Care Assistant to join the Frailty Team, delivering care to frail and elderly patients across Weymouth and Portland with home visits and assessment in both care homes and private residences.

You will work with GP surgeries and the Integrated Community Team, supporting monitoring visits, care planning, and clinical tasks under supervision. The role emphasises communication, accuracy in records, and patient-centred care.

Qualifications

• 5 GCSEs at C/4 or above including Maths and English (or relevant experience).
• Care Certificates (Desirable).
• Essential NHS/Primary Care experience and ability to work under own direction.

Responsibilities

• Support clinical care for identified frail/complex patients in their homes per care plan.
• Communicate effectively with patients, carers and health professionals.
• Assist with ongoing support for patients, families and carers to manage frailty and chronic conditions.
• Identify deterioration and refer promptly to minimise risk of hospital admission.
• Maintain accurate, timely patient records and share care appropriately.
• Undertake delegated clinical tasks after competency-based training (e.g., phlebotomy, ECG, BMI/BP, urinalysis).
• Develop and maintain relationships with health/social care colleagues to ensure integrated service delivery.
• Demonstrate autonomy in duties and adhere to information governance protocols.
• Contribute to service development and participate in team meetings.
