Job summary
Provide CIC in Chelmsford is recruiting a Band 7 Nurse with a non-medical prescribing qualification and an assessment/consultation course to join the Frailty Virtual Ward. The post supports delivering comprehensive geriatric assessments in patients’ homes and coordinating with UCRT and MDTs to prevent unnecessary hospital admissions.
The role requires at least 3 years of community experience, leading a multidisciplinary team, and driving project outcomes to improve frailty care and discharge
Qualifications
- Band 7 nurse with non-medical prescribing qualification.
- Assessment and consultation course completed.
Responsibilities
- Undertake comprehensive geriatric assessments for patients under frailty care and formulate personalised management plans with the MDT.
- Attend rota for virtual GP calls to discuss patients and liaise with the Frailty Consultant for complex medical management.
- Lead on developing the skills of registered staff and support staff, and train junior colleagues to maintain competencies; manage discharges with the MDT.
- Inspire others through action, challenge traditional practices, and promote innovative problem solving among staff.
- Agree outcomes for people with frailty and complex co-morbidities to reduce hospital admissions and long-term residential care.
- Develop innovative ways to increase patient flow in the frailty virtual ward, collaborating with UCRT.
