Job summary

Qualifications

• NMC registration required for Band 6 level practice.
• Experience in community health and frailty care is desirable.
• Ability to manage complex care plans and coordinate multidisciplinary teams.

Responsibilities

• Assess, treat and manage patients with complex acute needs in the community.
• Develop and review care plans; deliver evidence-based interventions.
• Coordinate care and respond to deterioration; maintain accurate records.
• Provide clinical leadership and supervise students/colleagues as needed.
• Contribute to service development, quality improvement and policy adherence.
