Job summary

The Single Neighbourhood Team Care Coordinator at Kent Local Medical Committee will act as the operational engine of the Single Neighbourhood Frailty Programme, coordinating proactive care for complex patients including those with severe frailty, and arranging CGAs and NEWS2 assessments across the PCN.

The post-holder will handle scheduling, administration, coding and data management to maximise clinician productivity and service delivery.

Qualifications

• Coordinating proactive care for complex care patients, including those living with severe frailty.
• Organising Comprehensive Geriatric Assessments (CGAs) and Structured Medication Reviews (SMRs).
• Carrying out NEWS2 assessments and data management to support clinicians.

Responsibilities
