Job summary

Sussex Community is seeking a Frailty Practitioner to deliver person-centered, advanced clinical care for people living with frailty. You’ll work within Integrated Neighbourhood Teams to support independence and prevent hospital admissions.

The role involves autonomous clinical decision-making, collaboration with GPs, geriatricians, social care and voluntary partners, and contributing to education and service development. Interviews are in late November.

Qualifications

• Advanced clinical assessment skills required.
• Ability to work autonomously with complex caseloads.
• Experience coordinating care across multiple services and teams.
• Strong communication and collaboration with multidisciplinary partners.

Responsibilities

• Undertake comprehensive geriatric assessments and develop personalised care plans.
• Manage a defined caseload of people living with frailty and complex needs.
• Identify deterioration early and prevent avoidable hospital admissions.
• Coordinate care across primary, community, acute, social care and voluntary sectors.
• Support safe hospital discharge and advise on care pathways.
• Provide specialist clinical leadership and education to colleagues.
• Advocate for patients and families to maintain independence.
