NHS Jobs is seeking a Qualified Advanced Clinical Practitioner to support GPs and the wider clinical team. The role involves autonomous assessment, diagnosis, treatment and ongoing management of patients, with opportunities to participate in the Primary Care Network Frailty Team and ward rounds at care homes.

The post requires flexible work across practice sites, with emphasis on evidence-based care, patient education, and collaborative teamwork in a high-quality NHS setting.

Responsabilidades

• Act as an autonomous practitioner, providing evidence-based, up-to-date and high-standard care appropriate to qualifications, experience and clinical skills.
• Organise referrals, further investigations and follow-up appointments as clinically appropriate, ensuring continuity of care and timely communication with relevant healthcare professionals.
• As an independent prescriber, maintain prescribing competence and follow local policies and protocols.
• Provide general and specific health screening for Practice patients within agreed protocols.
• Undertake day-to-day management of chronic disease management programmes, interpret blood results and investigations within scope of practice.
• Collaborate with GPs and multidisciplinary team to ensure safe, effective and coordinated care.
• Contribute to the Primary Care Network Frailty Team, including weekly ward rounds at care homes and research opportunities.
• Advise patients on general healthcare, promote education and preventative healthcare initiatives.
• Maintain accurate records of all patient contacts and clinical decisions, and participate in audits and meetings.
• Maintain CPD plans and professional registration, ensuring indemnity arrangements and training.
