Job summary

Selby Town PCN seeks an experienced Advanced Clinical Practitioner to join the Ageing Well Team, delivering proactive, person-centred care for adults with frailty in patients’ homes.

The postholder will undertake CGAs, coordinate care plans, and work closely with occupational therapy, physiotherapy, and care coordinators. Autonomous, flexible, and aligned with best practice in frailty care are essential.

Qualifications

• Significant post-registration clinical experience in advanced or senior roles.
• Experience delivering patient education and promoting self-directed care.
• Experience with frailty and long-term conditions in community or primary care settings.

Responsibilities

• Visit clients meeting Frailty criteria in their homes.
• Assess health needs of frail patients and plan care.
• Undertake and coordinate Comprehensive Geriatric Assessments (CGA).
• Develop person-centered, strengths-based care plans with patients and carers.
• Provide clinical leadership and support to care coordinators.
• Participate in MDT meetings to discuss patients and care plans.
