Job summary

Promni Health is seeking an experienced Advanced Clinical/Nurse Practitioner to join the Primary Care Network, delivering proactive, person-centred care to care home residents living with frailty and complex health needs. You will perform advanced clinical assessments, provide diagnosis, prescribing and management within PCN pathways, collaborating with GPs, pharmacists and district nurses.

Qualifications

• Masters degree in advanced practice is essential.
• Proven advanced clinical practice across clinical governance and CPD.
• Experience delivering proactive, person-centred care in care homes.

Responsibilities

• Provide advanced clinical assessment and autonomous decision-making for frail care home residents.
• Prescribe safely within PCN care pathways and local policies.
• Lead and support MDT meetings and care home rounds.
• Contribute to Personalised Care Plans and Advance Care Planning.
• Promote governance, documentation and quality improvement.
