Pioneer Medical Recruitment is working with Primary Care Network in and around Peterborough to find an experienced Frailty Nurse for a 3-month block booking. This is a sickness cover role working across the surrounding rural communities, supporting older and frail patients in their homes and care settings.
You'll carry out Comprehensive Geriatric Assessments (CGAs) for frail and elderly patients, working closely with GPs, the wider PCN team and community services. Your work will include identifying frailty, reviewing medication and functional needs, building personalised care and support plans, and helping to reduce avoidable hospital admissions.
You'll be a registered nurse with an active NMC/ HCPC PIN and solid experience in frailty or care of older people, including confidence in completing CGAs independently. Community or primary care experience is ideal, and you'll need a full UK driving licence and access to a car, as the role covers a wide rural area. An Advanced Nurse Practitioner with frailty experience would also be welcome to apply. Non-medical prescribers are encouraged to apply, with rates reflecting prescribing qualifications.
This is a well-defined 3-month booking with a supportive PCN team, a competitive hourly rate, and the chance to make a real difference to older patients in rural communities.
