Job summary

West Suffolk Clinical Commissioning Group is seeking a Frailty Practitioner to deliver autonomous clinical assessments, diagnosis, and treatment for frail and vulnerable patients in their own homes and local settings.

The post holder will work within SS9 South PCN, collaborating with GP practices, community teams, and social care to prevent hospital admissions and support timely discharge where appropriate.

Qualifications

• HCPC/NMC/Prescriber registrations are essential.
• Experience with frailty and urgent community-based care is required.
• Ability to lead autonomous clinical decisions and care planning.

Responsibilities

• Undertake urgent same-day frailty assessments.
• Prevent avoidable hospital admissions through rapid assessment and treatment.
• Conduct home visits and assess environmental risks.
• Collaborate with GP Practices, INT, Community Services and Social Care.
• Develop personalised care plans and anticipatory care discussions.
