Job summary

Ability Housing Association in Hillingdon offers a 40-hour per week role in our Floating Support Team. You will provide housing-related support to adults with mental health issues in Hillingdon, in homes and community settings.

The role emphasises a person-centred, recovery-based approach, helping customers gain independence and access services. Driving licence and car access are essential for this role, with a Monday–Friday schedule.

Qualifications

• Ability to work with adults with mental health issues.
• Strong communication and interagency collaboration.
• Full UK driving licence and access to a car are essential for this role.

Responsibilities

• Deliver housing-related support to customers in their homes and in the community, including helping develop independent living skills, maximise benefits, attend health appointments, engage in social and learning activities, volunteering and community engagement.
• Assist customers with finding and securing alternative accommodation.
• Support customers in setting up and maintaining a home/tenancy, including maintenance, rent and arrears, and being a good neighbour.
• Work with 13 customers using a strength-based, person-centred approach; manage up-to-date support plans and risk assessments.
• Minimise unplanned hospital admissions by offering emotional support and signposting to health services.
