Responsibilities

• Provide housing related support to customers, including help with maintaining or accessing housing and welfare benefits.
• Build positive, trusting relationships to enable customers to thrive with person-centred support.
• Help customers advocate for themselves and focus on what matters to them.
• Digitally monitor and record housing related activities and outcomes following support plans.
• Work as part of a team, upholding Hafod’s standards and safeguarding responsibilities.
• Proud member of the Disability Confident employer scheme.
