Job summary

Tai Pawb is seeking two full-time Floating Support Workers on a fixed-term maternity cover in Monmouthshire. You will visit people in their homes and the local community, manage your own caseload and diary, and provide person-centred support to help individuals live autonomously with dignity and stability.

You will work with people facing diverse needs, including substance use, build trusted relationships, create support plans and risk assessments, and signpost to specialist services while

Qualifications

• Driving licence and access to a car are required.
• Passion to help people live as independently as possible.
• Good initiative and ability to work alone as well as in a team.
• Ability to deal with conflicting and difficult situations.

Responsibilities

• Visit people in their homes across Monmouthshire.
• Build trusted relationships and set clear goals.
• Create support plans and complete risk assessments.
• Provide budgeting, benefits, health, housing and tenancy advice.
• Help address rent arrears and access grants.
• Work closely with Housing Options and partner agencies.
• Signpost to specialist services and attend appointments where required.
