Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Band 3 Community Healthcare Assistant for our Neighbourhood Teams. The post is six months fixed-term and part-time, with varied shifts from 07:30 to 18:30 across rostered patterns, supporting patients in their own homes.

You will work under nurses and therapists to deliver person-centred care, perform delegated clinical tasks, and contribute to care planning within a supportive team across Worcestershire.

Qualifications

• Band 3 Healthcare Support Worker role in NHS.
• Experience delivering person-centred care in patients' homes.

Responsibilities

• As a Band 3 Healthcare Support Worker, you will deliver high-quality, person-centred care in patients' own homes.
• Under supervision of registered nurses and therapists, perform delegated clinical tasks per care plans.
• Support personal care, daily living activities, record observations, and promote safety and dignity.
