Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Senior Community Nurse to join the Neighbourhood Teams in Pershore and Upton. The role covers home visits from 07:30 to 18:30 across the local area, with a focus on end-of-life care, wound care, and symptom management for adults 18+.

You will lead on care planning, teaching junior staff, and participate in teaching student nurses, while travelling between sites with a full UK driving license.

Qualifications

• Registered nurse with community experience.
• Evidence of ability to manage caseloads.
• Willingness and ability to travel across the local area.

Responsibilities

• Provide home visits for adults 18+ requiring nursing interventions.
• Lead team meetings, care planning, and evaluation; handover.
• Support nurse students pre and post registration.
• Oversee patient care decisions and clinical outcomes.
