You will complete and review person-centred care plans, conduct initial assessments and reviews, and participate in on-call rotas to support operational needs.

Qualifications

• Experience leading care teams in a health or social care setting.
• Ability to develop and review person-centred care plans and risk assessments.
• Strong understanding of compliance standards and safeguarding.
