Wye Valley NHS Trust is seeking a motivated Community Support Worker for the Falls Response Service within the Urgent Community Response Team. The role includes rapid home assessments, safe patient handling, and responding from the Community Referral Hub (CRH) with a 45-minute response target.

A driving licence is essential. You will join a multidisciplinary team, handling calls, processing referrals on EMIS, and supporting patients to regain independence.

Qualifikationen

• Experience working in a community setting is desirable.

Aufgaben

• Attend patients' homes within a 45-minute target after CRH contact.
• Assess safety and support patient with transfers using equipment.
• Provide basic care and follow manual handling guidelines.
• Complete the IStumble evaluation form in EMIS.
• Process referrals and onward referrals, with follow-up calls as needed.
• Support the wider CRH team and handle calls from community services and HCPs.
• Utilise multiple IT systems and type efficiently.
