Wye Valley NHS Trust is seeking a compassionate Community Support Worker for the Falls Response team in Hereford. The role offers full-time or part-time options with flexible working, including 12-hour shifts and one weekend per month.

You will respond to falls in the community, assess and support recovery at home, and work within the CRH call handling hub. The post requires a driving license and ability to work across a rota including day and night shifts.

Competenze

• Experience in Health/Social Care or related environment.
• Experience working in a community setting.
• Care certificate is desirable.
• CPD Diary demonstrating levels of learning and reflection.

Mansioni

• Attend patients' homes within a 45 minute target after a referral.
• Assess safety and plan care with equipment where needed.
• Support basic care requirements and manual handling as required.
• Complete the EMIS patient referral and any onward referrals.
• Handle calls and referrals on the EMIS system, and support the wider CRH team.
• Be adaptable to new processes and maintain a positive attitude.
