HCRG Care Group in Dartford is seeking a Healthcare Assistant to join the Falls Prevention Team. You will support home-based rehabilitation, assist with exercises, and help deliver group activities in the community.

The role involves visiting patients, promoting independence, and working with Physiotherapists and Rehabilitation Assistants. Driving licence and access to a vehicle are essential for community travel.

Qualifications

• NVQ Level 2 in Care or equivalent qualification.
• Experience in health, social care or rehabilitation.
• Strong communication and interpersonal skills.
• Ability to work in a team and independently when needed.
• Good organisational and time-management skills.
• Compassionate, patient-centred approach.
• Respect for dignity, privacy and confidentiality.
• Good IT skills with electronic systems.
• Ability to recognise concerns and elevate information.
• Willingness to learn and develop new skills.

Responsabilités

• Support the delivery of strength and balance classes.
• Visit patients at home to support exercises and wellbeing.
• Assist with mobility and daily living equipment reviews.
• Support rehab programmes created by clinicians.
• Encourage patients to meet rehab goals and stay independent.
• Provide information and practical guidance to patients and carers.
• Participate in health promotion and wellbeing activities.
• Monitor and report changes to the clinician as needed.
• Maintain accurate and timely patient records.
• Provide admin support to the team as required.
• Attend team meetings and training; develop professionally.
• Work independently on delegated visits within a team.
• Promote patient safety, dignity and wellbeing at all times.
