Job summary

Northern Health and Social Care Trust is seeking a Falls Assistant Practitioner (Temporary) to support the Falls Service across the Antrim area. The role requires carrying out multifactorial falls assessments, delivering prescribed exercise programmes and providing prevention advice.

The post is Band 4, part-time (22½ hours per week) with a pro-rata salary and a focus on collaborative care within the Falls Prevention Team. Experience in falls risk management is desired.

Qualifications

• Five GCSEs (A-C) including English or equivalent.
• Current full UK driving licence and access to a car on appointment.
• Excellent verbal and written communication skills; able to work in a team.

Responsibilities

• Carry out multifactorial falls assessment for referred clients.
• Provide falls prevention advice per protocols and deliver therapeutic exercise programmes.
• Make onward referrals to internal/external services and maintain accurate records.
