Job summary

The Assistant Practitioner in Community Therapy and Pathway 1 will work with colleagues to prevent hospital admission and support post-hospital discharge, while improving ADL function and rehabilitation in the community. You will communicate with patients and carers, tailoring your approach as required and ensuring high professional standards.

Induction includes full training and monthly clinical supervision; applicants pay for DBS checks with deductions from salary during the first two months.

Qualifications

• Falls prevention and rehabilitation awareness
• Experience in community work (desirable)
• Ability to communicate with members of the public and adapt communication style

Responsibilities

• Help prevent hospital admission and support post-discharge care
• Improve ADL function and provide care to patients and carers on waiting lists
• Maintain accurate records and escalate safeguarding concerns
• Support the team in all aspects of the service with supervision during induction
