NHS Jobs in Dartford, England, seeks a Healthcare Assistant to join the Falls Prevention Team. The role supports people at risk of falls, delivering home-based exercise, wellbeing visits and group activities with physiotherapists and rehabilitation staff.

You will help patients stay independent, monitor changes, and contribute to care planning within a supportive multidisciplinary team.

Formación

• NVQ Level 2 in Care or an equivalent qualification.
• Experience in health/social care or rehabilitation is desirable.
• Strong communication and interpersonal skills.

Responsabilidades

• Visit patients in their homes to support exercise programmes.
• Support delivery of strength and balance activities and wellbeing interventions.
• Assist with rehabilitation programmes alongside the team.
• Maintain accurate patient records and contribute to health promotion.
• Provide information and practical guidance to patients, relatives and carers.
• Attend meetings and work within a multidisciplinary team.
