Job summary

CCH Group is seeking a Team Leader for Care By Us in Wolverhampton. This full‑time role runs Monday to Friday, 9am–5pm, to support the delivery of high‑quality care and supervise care workers at the Extra Care scheme.

You will conduct needs and risk assessments, develop person‑centred care plans with tenants and carers, and lead a team through coaching and supervision, contributing to continuous improvement and staff recognition programs.

Qualifications

• Experience carrying out needs and risk assessments to inform care delivery.
• Ability to develop person-centred plans of care with tenants and their carers.
• Experience leading a team through shadowing, supervising, instructing and supporting care workers in Extra Care schemes.

Responsibilities

• Assess and review individual services to ensure high quality care delivery.
• Provide practical supervision, instruction and support to care workers.
• Lead the team within the Extra Care scheme and promote person-centred care for tenants.

If you are passionate about supporting the delivery of the highest quality care support services by carrying out a range of functions, including the assessment and review of individual services and the practical supervision, instruction and support of care workers then we would love to hear from you.

Qualifications

You need experience in carrying out needs and risk assessments and developing person-centred plans of care delivery for, and in partnership with, individual tenants and their careers. Also experience in leading a team including shadowing, supervising, instructing and supporting care workers in the Extra Care scheme.
