Job summary

City & County Healthcare Group Ltd's Care By Us division in Hertford is seeking a Team Leader to oversee care delivery in the Extra Care scheme. This full-time permanent role involves supervising care workers, conducting needs and risk assessments, and developing person-centred care plans in partnership with tenants and carers.

The role offers the chance to lead a dedicated team, drive high-quality services, and contribute to a supportive care environment within the Hertford branch.

Qualifications

• Experience in carrying out needs and risk assessments and developing person-centred plans of care delivery for, and in partnership with, individual tenants and their carers.
• Experience in leading a team including shadowing, supervising, instructing and supporting care workers in the Extra Care scheme.

Responsibilities

• Provide practical supervision, instruction and support to care workers.
• Assess and review individual services to ensure high-quality care delivery.
• Develop person-centred care plans with tenants and their carers.
• Lead and support the Extra Care team to deliver services effectively.

If you are passionate about supporting the delivery of the highest quality care support services by carrying out a range of functions, including the assessment and review of individual services and the practical supervision, instruction and support of care workers then we would love to hear from you.

Qualifications

You need experience in carrying out needs and risk assessments and developing person-centred plans of care delivery for, and in partnership with, individual tenants and their careers. Also experience in leading a team including shadowing, supervising, instructing and supporting care workers in the Extra Care scheme.
