Job summary

CCH Group in Wolverhampton is seeking a dedicated care coordinator to support the delivery of high-quality care services. The role involves needs and risk assessments, developing person-centred plans, and supervising care workers in the Extra Care scheme.

Based at Broadway Gardens, the position is Monday–Friday 9am–5pm with a salary of £24,784.50, offering leadership opportunities within a trusted sector leader and access to top experts and innovative systems.

Qualifications

• Experience carrying out needs and risk assessments and developing person-centred care plans in partnership with tenants and carers.
• Experience leading a team, shadowing, supervising, instructing and supporting care workers in Extra Care schemes.

Qualifications

You need experience in carrying out needs and risk assessments and developing person-centred plans of care delivery for, and in partnership with, individual tenants and their careers. Also experience in leading a team including shadowing, supervising, instructing and supporting care workers in the Extra Care scheme.
