Job summary

London Care is seeking a Team Leader to support the delivery of high-quality care services in the Extra Care scheme at our Chatham location (ME4 6FE). You will assess needs, develop person-centred care plans and supervise, train and support care workers to deliver exceptional support to tenants.

Qualifications

• Experience carrying out needs and risk assessments.
• Developing person-centred plans of care for tenants and their carers.
• Experience leading a team including shadowing, supervising, instructing and supporting care workers in Extra Care.

If you are passionate about supporting the delivery of the highest quality care support services by carrying out a range of functions, including the assessment and review of individual services and the practical supervision, instruction and support of care workers then we would love to hear from you.

Qualifications

You need experience in carrying out needs and risk assessments and developing person-centred plans of care delivery for, and in partnership with, individual tenants and their careers. Also experience in leading a team including shadowing, supervising, instructing and supporting care workers in the Extra Care scheme.
