Job summary

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner to join the Home Visiting Service in the UK. You will work autonomously in patients' homes, using advanced assessment, diagnostic and prescribing skills to provide safe, effective urgent care in the community.

You’ll care for patients across General Practice, including children, older people, and those with palliative needs, ensuring patient-centred care and timely escalation when required.

Qualifications

• Hold current professional registration with NMC, HCPC or GPhC.
• Masters Degree / Level 7 qualification in Advanced Clinical Practice.
• Enhanced Assessment and Consultation qualification at Level 7.
• Minor Illness qualification at Level 7.
• Registered Independent Prescriber.
• Have advanced assessment, examination, diagnosis and treatment skills.
• Be capable of autonomous practice with strong clinical reasoning.
• Be flexible to work unsociable hours, weekends and Bank Holidays.

Responsibilities

• Provide home-based, face-to-face assessment, diagnosis and treatment.
• Prescribe safely within clinical guidelines and manage ongoing medications.
• Maintain accurate clinical records and participate in audits and CPD.
• Escalate to GPs/Clinical Consultants when higher input is needed.
• Collaborate with primary care, community teams and social care to ensure continuity.
• Deliver patient-centred care to diverse populations, including palliative needs.
