Job summary

Integrated Care 24 Ltd is seeking an Advanced Clinical Practitioner to join its Home Visiting Service across Sussex. You will assess patients in their own homes, provide urgent care, and prescribe where appropriate, supporting care plans and referrals.

The role requires autonomous decision‑making, strong clinical reasoning and the ability to work within IC24’s governance framework. The post offers permanent hours up to 37.5 per week, with a pro rata salary of £54,140.87 per annum and benefits

Qualifications

• Masters Degree / Level 7 Advanced Clinical Practice
• Enhanced Assessment and Consultation Level 7
• Minor Illness qualification Level 7

Responsibilities

• Assess patients in their own homes and provide advanced urgent care.
• Prescribe and manage treatment at home within clinical guidelines.
• Maintain accurate clinical records and CPD with supervision and governance.
