Job summary

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner for the Home Visiting team in Ashford and surrounding locations. You will assess and treat patients in their own homes, using advanced clinical reasoning to provide safe, effective care and appropriate escalation when needed.

You will work autonomously, managing a diverse caseload including urgent care, palliative and end‑of‑life needs, while collaborating with GPs and community teams to keep patients safely in their homes

Qualifications

• Registered clinician with NMC/HCPC or GPhC and full registration status.
• Masters Degree / Level 7 qualification in Advanced Clinical Practice.
• Enhanced Assessment and Consultation qualification at Level 7.
• Experience and qualification as an Independent Prescriber.

Responsibilities

• Provide advanced urgent care in patients' homes with autonomous decision‑making.
• Assess, diagnose, prescribe and manage treatment in home settings.
• Coordinate with GP, clinical colleagues and other services to ensure continuity of care.
• Deliver care within bounds of professional registration and governance frameworks.
