Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Band 4 Senior Rehabilitation Assistant to support multidisciplinary rehabilitation programmes. The role includes delivering home visits and care-home based interventions, with regular supervision from senior therapists.

You will assist in discharge planning, manage a varied caseload, and contribute to service development and quality improvement initiatives.

Qualifications

• Experience delivering rehabilitation programmes in multidisciplinary teams.
• Ability to contribute to discharge planning and home-based interventions.
• Capability to assess patients in their own homes and escalate concerns when needed.
• Support service development and participate in quality improvement initiatives.

Responsibilities

• Coordinate and conduct rehabilitation programmes (1:1 and group) in care home settings and at patients’ homes.
• Collaborate closely with physiotherapists and occupational therapists on patient progression.
• Identify signs of deterioration and escalate to the physiotherapist or OT.
• Manage diaries for classes and patient appointments.
• Provide training support to wider rehabilitation staff on safe and effective exercise.
