Southern Health NHS Foundation Trust is seeking Healthcare Support Workers to join our Enhanced Recovery Service. You will support people in their own homes after illness or hospital admission, helping them regain confidence and independence by promoting daily living skills and reablement.

Working closely with patients, families and health and social care colleagues ensures personalised care focusing on what matters most to the individual.

Description du poste

We are looking for caring, motivated and enthusiastic Healthcare Support Workers to join our Enhanced Recovery Service.

Working as part of a multidisciplinary team, you will support people in their own homes following illness, injury or hospital admission to regain confidence, maximise independence and achieve their personal goals. You will provide practical reablement support, encouraging individuals to develop daily living skills and maintain their independence wherever possible.

You will work closely with patients, families, carers and health and social care colleagues to deliver person-centred care that focuses on what matters most to the individual. The role requires good communication skills, a compassionate approach and the ability to work both independently and as part of a team.
