St Anne’s Community Services is seeking a Support Worker for Greenwood, Leeds on a part-time basis. Starting salary £12.71 per hour, rising to £12.91 after probation and £13.45 after 12 months, with pro rata annual pay up to £26,639. The role involves delivering person-centred support and helping clients live independently.

Responsibilities include care planning, promoting client wellbeing, and maintaining records. Must be flexible across shifts and hold or undertake a Care Certificate.

Qualificações

• Fully flexible and able to cover all shifts including days, evenings, nights and sleep-in shifts.
• Desirable to have a current driver licence.
• Care Certificate or willingness to undertake it.

Responsabilidades

• Prepare person-centred support plans reflecting clients' views and aspirations.
• Support clients to live independently and engage in activities and hobbies.
• Communicate openly and transparently with clients and team members.
• Assist with personal care when required while respecting client dignity.
• Maintain accurate records and daily notes with client involvement.
• Carry out household duties such as cooking, cleaning and laundry.
