Qualifications

• Recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
• Registered with Social Work England.
• Experience in urgent and emergency care settings or similar environments.
• Excellent communication and interpersonal skills; ability to work effectively in a fast-paced environment.
• Demonstrated ability to collaborate with diverse teams and stakeholders.

Responsibilities

• Conduct assessments of patients’ social, emotional, and practical needs in urgent and emergency care settings.
• Collaborate with multidisciplinary teams to develop and implement care plans addressing patient needs.
• Provide emotional support and guidance to patients and families, explaining options and rights.
• Connect patients with community resources to aid hospital discharge and transitions.
• Maintain accurate documentation in accordance with policies and procedures.
