Job summary

Caritas Recruitment is seeking an Advanced Practitioner for the Urgent and Emergency Care team in Nottinghamshire. You will lead complex assessments, develop care plans, and collaborate with multidisciplinary colleagues to secure timely, person-centered support for individuals in urgent situations.

The role requires registration with Social Work England, a recognised social work qualification, and at least three years of post-qualification experience in urgent care.

Qualifications

• BA/MA in Social Work or equivalent qualification.
• Registered with Social Work England.
• Minimum of three years post-qualification experience in urgent and emergency care.
• Strong assessment and analytical skills under pressure.
• Excellent communication and interpersonal skills focused on person-centered care.

Responsibilities

• Conduct comprehensive assessments of individuals in urgent and emergency situations.
• Develop, implement and review care plans with service users and families.
• Provide guidance and supervision to junior staff and students.
• Engage with external agencies to coordinate care.
• Maintain accurate records in line with regulatory standards.
