This is an exciting opportunity to join Preparing for Adulthood (Adult Social Care), a multi-specialist team at Nottinghamshire County Council, focusing on assessment and support planning for young people transitioning into adulthood. You will work in a strength-based manner, aiming to make a real difference in the lives of young people. Your role will be pivotal in helping them achieve their aspirations during this crucial time.

As a Social Worker, your key responsibilities will include:

• Conducting assessments and developing support plans for young people.
• Collaborating with Children’s Social Care, Education, and Health services.
• Supporting young people in achieving their goals and outcomes.
• Ensuring compliance with adult legislation and safeguarding practices.
• Maintaining accurate records and producing reports.

• Undertaking health and safety risk assessments.

To thrive in this role, you will need:

• A Social Work qualification (registered with Social Work England).
• Knowledge of community care services and relevant legislation, including the Care Act 2014.
• Proficiency in using IT applications for case management and reporting.
