Job summary

Qualifications

• Recognised Social Work qualification (e.g., BA/MA in Social Work)
• Current registration with Social Work England
• Proven experience in community health and social care settings
• Strong interpersonal and communication skills
• Excellent organisational and time management skills

Responsibilities

• Conduct comprehensive assessments of clients’ needs and circumstances.
• Develop, implement, and review care plans in partnership with clients and families.
• Provide emotional and practical support to clients, ensuring their voices are heard.
• Engage with community resources and services to facilitate access to support.
• Maintain accurate case records in line with local policies and regulations
