Snapshot:
  • Role: Nurse Case Manager
  • Hours: Full-time, 35+ hours per week, with on-call duties on a rota basis.
  • Location: Across Lancashire, primarily home-based with some travel. Must possess a full UK driving license and own vehicle.
Role Overview:

This is an exciting opportunity to be a key part of the Routes family, ensuring high standards of care.

As a Nurse Case Manager, you will:

  • Set and monitor standards and quality of care for service users.
  • Maintain high standards of record keeping in line with NMC, legislation, and local standards, facilitating effective communication with multi-professionals.
  • Promote a culture of person-centered care within the community and across Routes Healthcare.
  • Manage a caseload of complex clients, overseeing their care holistically and clinically.
  • Develop, review, and implement care plans and risk assessments in accordance with CQC regulations, supporting non-qualified staff development.
  • Oversee and manage staff within care packages.
  • Ensure ongoing competency of staff through observations, supervisions, and support sessions.
  • Coordinate and lead transitions, start-ups, and continuity of complex care packages, managing clinical training and maintaining skill mix for safe practice.
  • Manage clinical interventions and staff training.

  • Registered Nurse (Band 6 or above) - Essential
