About the Role:
As a Physical Health Nurse in the SWK CMHT, you will:

- Mentor, supervise, and line manage staff: Providing leadership, guidance, and support to your team within the SWK CMHT, fostering a positive and productive work environment within the Trust's framework.
- Assess, act on, and report any safeguarding concerns: Ensuring the safety and wellbeing of our service users within the SWK CMHT by adhering to the Trust's safeguarding policies and procedures.

About the Role:
As a Physical Health Nurse in the SWK CMHT, you will:

Main duties of the job
- To coordinate and make recommendations to the care plan process

- To assess, act on and report any safeguarding concerns.
- Promoting a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance.
- Acting as a role model and supporting new team members through demonstration and explanation of tasks and techniques.
- Facilitating, supporting and assessing staff undergoing personal and/or professional development.
- Undertake and maintain all essential training to role.
- Working confidently in a variety of environments according to changing service needs which present different challenges whilst continuing to act in a professional manner.
- Balancing all aspects of the job role across the 7-day working pattern.
- Being flexible and responsive to competing demands to ensure safe delivery of services.

Detailed job description and main responsibilities
- To work within the community mental health setting as a valued member of the multi-disciplinary team providing and contributing to high quality nursing care to service users with severe mental health problems.
- Be responsible for the risk assessment, planning, implementation and evaluation of emotional and physical care needs for service users, and to be fully aware of the care needs of service users over a 24hour period.
- Monitor, report and maintain a healthy, safe and secure workplace by knowing and taking account of Health and Safety at work Act (1974), reporting of injury, diseases and dangerous occurrences regulations (1965), control of substance Hazardous to health (1985) European H&S Regulations (1992) and adhere to Trust policies and local services protocol

Essential criteria
- Nurse and NMC registration.
- Experience within acute mental Health services.
