Job summary

The Methodist Church in Britain is seeking an HCPC-registered Occupational Therapist to join the Aylesbury Rapid Response and Intermediate Care service, providing community-based assessment and care across Buckinghamshire. The role emphasises triage, planning, case management and coordination to promote independence and support early discharge.

You will work with a multi-disciplinary team, develop clinical pathways, and lead care planning and delivery.

Qualifications

• Degree or diploma in occupational therapy.
• Recent continuing professional development.
• Postgraduate clinical training (e.g. advanced history taking and assessment).
• Training in long term conditions, respiratory disease, cardiac disease, trauma, orthopaedics and end of life.

Responsibilities

• Provide triage, comprehensive specialist assessment, review and evaluation of patients' and carers' needs using OT skills and experience.
• Plan programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys.
• Allocate workloads to members of the Rapid Response & Intermediate Care team.
• Work with a range of professionals to ensure care is appropriate and delivered in a timely manner.
• Demonstrate professional skills and competence in leadership, care planning and co-ordination, and care and treatment delivery.
• Support the development of the clinical pathway.
